Insurance
Requirements |
|
|
Worker's
Compensation is required in the state of Oklahoma (this is not required on
our Texas properties). A certificate with
valid dates must be submitted before we will consider any firm. You will
need one of the following: |
|
|
- A valid Non-Coverage card issued by
the Oklahoma Department of Labor will be accepted until 7-15-06.
- If you are a family owned and operated
business, each family member must sign a disclosure statement on each
invoice for which you receive payment stating (in summary) that you
are a family of five (5) or fewer and you will waive your rights to
worker's compensation insurance.
- Liability Insurance of $500,000 or
more, depending on the work being performed.
- Some available work could also require
a surety bond. Email Jason

Other
Requirements |
|
|
All current and prospective
Independent Contractors must have a W-9 on file with D&S Management. Click

A
signed D&S Vendor Agreement. These Agreements are available for review
at the time of bidding or of submission of documents on this page. |
|
|
Unconditional
Lien Waivers - signed by the appropriate members of your company when the
work is complete. |
|
|
|
|
|
Payment
Terms |
|
|
All
vendors are paid on a 30-day term from the date the invoice is received at
the corporate office. Some jobs might be available for immediate payment
if the total cost exceeds $1,500. |
|
|
If
payment has not been received within 30 days, please contact our Accounts
Payable Department. |
|
|
|
|
|

No
payment will be made if D&S Management, Inc. does not have current
valid insurance certificates or any of the documentation described above. |
